Active MC / DOT Authority

Must be active for at least 30-90 days depending on broker criteria.

Signed W-9 Form

Dated within the current tax year.

Certificate of Insurance (COI)

$100k Auto Liability & $100k Cargo Insurance listing Truckora as Certificate Holder.

Notice of Assignment (NOA)

If you utilize a factoring company for quick payouts.
